  • Appeals Letters
  • Insurance Correspondence

The Spine Council Administration will identify Proper Submission and Appeals Process to Accept or Decline for further review.

If Declined, reasons for decline will be indicated on response form:

  • Documentation Incomplete
  • Submission Process Incorrect
  • Appeal Process Incorrect
